Description: We are US based looking to outsource our requirement of Web and Software Development Service. We are looking for web based information system.

Scope:
1.Web-based data collection using php application framework.
2. Data storage in mySQL or postgreSQL.
3. Data structured in accepted normalized method.
4. All code and data structures documented.
5. Application should require no add-ons unless such are well-established, likely to be available for the known future, have no special maintenance requirements, and no ongoing licensing costs.
6. Application will require a typical log-in infrastructure
7. Application should have a user management system to add, delete or edit users and their permissions as well as manage passwords by one single administrator
8. Application should be “email-ready”. That is, they should have the entire infrastructure in place to send out data-triggered email alerts, although none are needed at present.
9. Application will be required to store Social Security numbers. Although this data does not need to be encrypted, all user interfaces and reports should display only the last four digits.
Application hosting, including DNS, web server, php and database hosting will be provided by us.
- Application: Participation Management System Elements
1 Description: Application will be used to store data concerning participants. Each participant will be affiliated with up to one cycle which will contain Benchmarks (that will change from cycle to cycle), and the date on which each participant achieves a benchmark will be recorded.
2. Data: Table structure will be along the following lines. In each case, additional field requirements may be added as needed.
a. Participation: i) Unique ID; ii) Contact information; iii) Demographic information like gender, Ethnicity/Race, DOB, etc. iv) Employment status like Employed (multiple choice: Part Time, Full Time, Unemployed) etc.; v) Cycle affiliation; vi) Append-only, date- and user-stamped narratives. vi) Criminal Record (yes/no); vii) Others
b. Employment History (single to multiple employers)
c. Cycles: i) Unique ID; ii) Descriptive information; iii) Important dates (start, end, etc)
d. Benchmarks and Benchmark Achievement
e. Placement and Follow-ups, etc.
3. User Interface (UI): UI will include a standard dashboard with options like i) Add new participant, new cycle; ii) Edit existing participant, existing Cycle; iii) Enter placement and follow-up information; iv) Reports; v) Edit user permissions and information (password changes)
4. Reports: A wide range and variety of data reports will be required. The MIS solution must provide the ability to design custom reports for future needs
